Track It, Trend It, Fix It — Rehospitalization Reports in SHP

May 27, 2026

This 3-part webinar series is built around "Track It, Trend It, Fix It." Each session gives you a focused, hands-on look at specific tools in IntelliLogix™ designed to help you monitor rehospitalization rates, understand the patterns behind them, and intervene at the resident level — before it's too late.

Session 1: The Big Picture

Wednesday, May 27, 2025  |  2:00 PM Central  |  30–40 minutes

In this kickoff session, we'll do a high-level walkthrough of the rehospitalization reporting suite: what each report measures, what question it's designed to answer, and when your team should be reaching for it. You'll leave with an understanding of how these tools fit together as a system — not just a collection of reports.
