A large, public home health company reached a point where it no longer had control of what was happening at the branch level because of its rapid growth, much of which was from acquisition. It was nearly impossible for executive management to know which agencies were performing well and which agencies needed to improve.

“Before SHP, our hospice organization was utilizing an EMR for clinical documentation and business office functionality. Even with an EMR, we had a difficult time collecting clinical, quality and outcomes data in a meaningful and timely fashion. For example, our QAPI process consisted of collecting multiple reports and then transferring information into meaningful data for QAPI meetings. This process was time consuming and inefficient. Now we use the QAPI dashboard in SHP to guide our QAPI meetings. The dashboard can be printed, brought to QAPI and PIP teams spend their time improving patient satisfaction and outcomes, instead of data collecting. We no longer keep quality and outcomes spreadsheets and do not use our resources to manually collect information. What I like the most about SHP is the real-time clinical information on our patients. Clinical leadership and MD’s have real-time information which enables them to make timely plan of care changes that improve symptom control, improve satisfaction and enables the patient to be managed in the place they call home, which is where hospice patients prefer to be. As a COO responsible for a large palliative and hospice operation, SHP enables me to run an Enterprise report that gives me valuable information about the overall business. Key operational information including admission location, discharge location, age of patients, diagnosis and LOS by organization and individual business unit allows a quick overview into the organization. Last and most important, the customer service is the best in the business. My staff raves about the online education, quick response and the willingness of SHP to create special reports and dashboards to meet our business needs. “Thanks for the information. We are a current subscriber to SHP services. I am VERY excited about the things we are able to do because of the reports we are able to generate. We recently subscribed to the Case Manager/ Clinician interface with CareAnyWare. I wish we have done this a long time ago. With these reports, along with the HHC top 20% numbers from Fazzi, we are developing a P4P system for our clinicians. We are integrating the Case Manager and Clinician Scorecards with the Training and testing from Fazzi to improve our outcomes. We will begin a P4P performance increase system on January 1st based on the Scorecards. This puts some skin in the game for the clinicians to get their scores up, and since we can give them the individual scorecards, we can use it to determine raises and bonuses. We believe the scorecard system gives them incentive to learn and improve. You can’t argue with the data. We have also begun an intensive program on monitoring the alerts and edits. We have explained to the staff how making sure these are corrected affects their scorecards, so it suddenly became more important to make corrections. I am looking forward to seeing how the HH‐CAHPS will work. We have been using your surveys anyway for a while and just moved to the new version. SHP for Agencies is well worth the money! It has saved us thousands of dollars financially and will be improving our outcomes over this next year with our new Scorecard program in place.”
